Here's all you need to do:
1) Fill out the printable order form (obviously) & include any special instructions that you may have. Please
INCLUDE EITHER YOUR TELEPHONE NUMBER OR EMAIL ADDRESS in case we need to contact you.
2) Send us any photos, pin ups, posters that you want in your video.
3) Also send us your musical selection either on audio or video tape.
ALL MATERIALS MAILED TO US WILL BE RETURNED TO YOU VIA 1st CLASS MAIL.
